With reference to my Insignia 2.0 CDTi estate (2010) I have had a few random occasions when my engine light has come on, gone into limp mode and engine ends up running like a bag of spanners. On inspection there appears to be no signs of overheating. Luckily i have a code reader in the car and have been able read the generic P2428 code and then to reset it to keep going.
I have been looking at the live data on my code reader and there only appears to be one anomaly which i will get into shortly.

From the information i have managed to accumulate, with difficulty i might add, the vehicle has two bank 1 sensors. Though i'm not sure which one is sensor 1. I have two sensors on front of engine the lower one being what is recognised as sensor 2 and from what i believe to be an o2 sensor at top.
My live data shows the following for theses sensors;
Catalyst temp sensor 1 (CATEMP11) reads 0 degrees all the time
Catalyst temp sensor 2 (CATEMP12) reads current temp value. (when connector disconnected reads 1000 degrees) Also when sensor 2 disconnected get a pending code of bank 1 sensor 2 temp too high.
My questions are if anyone has had any experience with this;
1. would a vast difference in temp between sensor 1 and sensor 2 possibly cause this intermittent fault?
2. Where is sensor one?
Im going to give the egr a clean at the weekend but i have monitored DPF temps etc and looked at the freeze frame data but nothing seems to be obvious. Any help would be appreciated.
